  3. WITH THE BEAD OF BLOOD ON THE END OF YOUR FINGER, SMEAR IT ON THE TOP OF THE PAGE.
  4. LET IT DRY AND PLACE IT IN AN ENVELOPE WITH A CHECK FOR $100 FOR THE TEST.
